CASE SHEET
CHIEF COMPLAINTS:
A 19 year male patient farmer by occupational came to the OPDwith chief complaints of swelling and pain in the right foot while walking , standing and after sitting for a long time.
HISTORY OF PRESENT ILLNESS:
Patient was apparently asymptomatic 8 months back then he started getting pain in the right ankle which subsided on medication.
He continued to have on or off pain 5 months.Later he went to an RMP doctor where he prescribed Medication but gradually pain and swelling increased .Then he visited a doctor where MRI was done which showed Infection for which surgery was performed.
HISTORY OF PAST ILLNESS:
No DM
No HTN
No Asthma
No Epilepsy
No CAD
Tb since 1 1/2 month
PERSONAL HISTORY:
Single
Appetite - normal
Diet - mixed
Bowl movement - regular
Micturition - normal
Sleep- normal

FAMILY HISTORY:
Not significant family history
ALLERGIES:
No allergy

No drug history
SURGICAL HISTORY:
Surgery was performed on the right foot
PHYSICAL EXAMINATION:
General examination:
Patient is conscious coherent cooperative
No pallor
no cyanosis
no lymphadenopathy
No clubbing
No pedal edema
